Welcome to Partners in Better Health! Partners in Better Health is a unique benefit that helps members and their dependents manage certain health conditions more effectively. Maintaining a healthy lifestyle can be especially challenging if you are managing a chronic condition. We want you to feel your best and give your best every day!

This care management program is designed to help people who are non-Medicare eligible and covered under the unions' medical plans to better manage certain chronic conditions, such as asthma, chronic obstructive pulmonary disorder (COPD), congestive heart failure (CHF), cancer and diabetes.

This program includes:

• Access to a Partners in Better Health Care Coordinator who will periodically call program participants to review their health status, answer questions, provide encouragement, and support participants in their efforts to manage their condition;
  
  
• A staff of professionals who will be available by phone to answer more complex clinical questions about participant conditions;
  
  
• Helpful education materials that will be sent to program participants. These educational materials are also available in the Information Center and Participant Information sections of this Web site;
  
  
• Health assessments that will be used to determine each participant's overall health status and identify areas where additional support and education may be required. Selected assessments are also available on this site in the Assessment Center.
