Participant Program Description
The Partners in Better Health program provides support, information
and services to help you better manage your condition. This valuable
program is like having a personal support team-registered nurses,
health educators and dieticians-all available to answer your questions,
educate you on a condition or diagnosis or help you better understand
your doctor's course of treatment. By participating in the program,
you may improve your ability to carry out daily activities and improve
your overall quality of life. The goal of this program is to help
all participants to enjoy good health, feel their best, and prevent
or delay serious health problems. By making essential self-care
activities a lifelong habit and becoming actively involved in management
of their health, all program participants can take charge of their
health!
Program participants will receive the following:
- Condition specific health risk assessment at least annually
to assess their healthcare status;
- Periodic educational materials following each telephone contact.
Participants may also request specific healthcare information;
- Toll-free access to a clinician available to answer complex
clinical questions; 24 hours a day, seven days a
week;
- Toll-free access to a Partners in Better Health Care Coordinator
who will review your health status and answer questions.
This is a participant education program. If you have urgent information
or concerns regarding your health care, please contact your healthcare
provider.
On this site, a copy of your participant rights and
responsibilities is available. Please take a moment to review
them. All participants have the right to voice a complaint or other
feedback related to the program. All of our staff is trained to
accept your information any time. We strive to protect your rights.

hours a day, seven days a week. You may also use this toll-free
number to file a grievance, complaint or feedback about the program.
