Take Care • Take Charge
MaternaLink Maternity Program

Welcome to the Take Care • Take Charge Condition Management Program, administered by Alere for ICI. This program provides support, information and services to help you better manage your asthma, congestive heart failure, chronic obstructive pulmonary disease, coronary artery disease, cancer, diabetes, and low back pain and improve your quality of life. It is being provided at no additional cost to you through ICI.

The program provides access to a Condition Management Program care coordinator who will periodically call you to review your health status, answer questions and provide encouragement. A staff of professionals will be available to answer more complex clinical questions about asthma, congestive heart failure, chronic obstructive pulmonary disease, coronary artery disease, cancer, diabetes and low back pain.

Using the Take Care • Take Charge Condition Management Program and this Website:

1. You may enroll in the program by phone or online. To enroll online, go to My Information and enter your personalized information* in this secured area. (Note-You will need the user ID and Password that are contained in the Announcement Letter you have received).

2. After enrolling, you may complete your initial assessment online or via phone with a Condition Management Program care coordinator. To complete your assessment online, click on the Assessment Center and the assigned assessments will be made available.

  • Multi-Condition Risk Assessment — provides an overview of your overall health status to assist program personnel in identifying ways to help you improve your health.

  • SF-8 — Provides an assessment of your current quality of life and where additional assistance could be helpful

  • Medical Barriers — Helps us to understand any individual issues that may make it difficult for you to manage your health.

3. Once you have completed your assessments, specific educational materials will be provided based on your results. These are available in the Assessment Center, after you've completed Step 2 above. They also will be mailed to you. Additional educational materials and tutorials are available in the Information Center and on the Participant Information page respectively (see left navigation bar).

If you have not already been contacted by Take Care • Take Charge staff and choose not to enroll online, you may enroll in this program by calling (888) 863-2262. Take Care • Take Charge care coordinators will be available Monday through Thursday, 8 a.m. to 11 p.m.; Friday, 8 a.m. to 9 p.m. and Saturday, 9 a.m. to 6 p.m. Eastern Standard Time.

* If you change your address or telephone number in the My Information Section, please be sure to notify your local HR benefits office of address or telephone changes.
