Participant Program Description

The Take Care • Take Charge Condition Management Program provides support, information and services to help you better manage your asthma, congestive heart failure, chronic obstructive pulmonary disease, coronary artery disease, cancer, diabetes and low back pain. This program will help you improve your overall quality of life, as well as your ability to carry out daily activitied.

Program participants will receive the following:

• Health status evaluation to determine your health status
  
  
• Periodic educational materials (available in Spanish as well as English) following each telephone contact
  
  
• Toll-free access to a clinician available to answer complex medical questions 24 hours a day, seven days a week
  
  
• Toll-free access to a care coordinator who will review your health status and answer your questions
